Emerge Aotearoa is a social services organisation providing a range of services to whānau, including Health and Disability, Youth and Transitional Housing. Our kaimahi all have a common goal - helping people realise their potential with the aim of supporting whānau to live their best life as independently as possible.

Te Tumu Waiora - te reo for 'to head towards wellness' - is a new model of primary mental health and addictions care and support which aims to provide all New Zealanders experiencing mental distress or addictions challenges with access to convenient, high quality, integrated and person-centred care and support. The model is being rolled out nationally over the next four years.

Te tūnga - The role

The Health Coach role aims to connect directly with patients to assist them to make any necessary changes to improve their health and wellbeing and work towards an achievable healthcare plan. The role involves being an integral part of the general practice team, working directly with individuals and their families to help improve their mental health and wellbeing.

The model puts mental health and wellbeing at the heart of general practice with focused roles, Health Improvement Practitioners and Health Coaches, working as part of the general practice team.

There's nothing average about this role and no two days are truly the same. You'll support patients with both nutritional/exercised based intervention and mental health recovery, via appointments run at either the clinic or at the patient's home. You'll run programmes alongside the Health Improvement and complete case notes and handovers with the practice staff.

While you will work to support patients of all ages, you'll predominately support people aged between 20 and 70.

This is a part-time vacancy working 20 hours per week Monday to Friday based at a medical practice in Ferrymead.

Māu - About you:

You'll come from a background in either nutrition, psychology, social work, or a related field, and have at least two years experience. You'll have the ability to build relationships and rapport easily, while maintaining professional boundaries. You'll also have a level five or higher qualification in a relevant field.

You'll need to be a strong effective communicator and have the ability to have patients open up in a limited timeframe. Our patients also come from an array of backgrounds so cultural competency and the ability to connect to people from different backgrounds and cultures are essential to this role.

You'll need to meet the Children's Act safety checking and our vetting requirements and have a full clean, valid driver's licence. You'll have strong computer skills and feel comfortable completing case notes.
